LONG ISLAND MUSIC EDUCATOR PERFORMANCE PRIZE ENTRY FORM

Completed applications must be received no later than August 15, 2025.

The winner will be announced no later than September 5, 2025.

If selected as the winner you agree to perform the concerto with the NSSO in concert on the evening of October 25, 2025 at Adelphi University Performing Arts Center, Garden City, New York.

I understand all NSSO concerts, program and repertoire are subject to change, and in the event NSSO has to postpone or alter the program of the October 25, 2025 concert, NSSO will make every effort to reschedule the winner's performance.
